It is more likely, however, that it was founded in the early 5th century by Sixtus III, whose name appears in the mosaics of the triumphal arch in front of the apse. The church had a nave and aisles, the nave more than twice as wide as the aisles, and there was a single apse. Monolithic Ionic columns supporting a continuous entablature divided the nave from the aisles; above these, clerestorey windows corresponded to the intercolumniations below. The windows were flanked by Corinthian pilasters aligned over the Ionic columns of the colonnade, and these were inset with a double tier of stucco colonnettes with fluting that spiraled right and left. Beneath each window was an aedicule encasing a mosaic panel. Until the 12th century, when Eugenius III (reg 1145–53) built the narthex, work on the church was mainly limited to maintenance. The first important changes were made in the late 13th century, when the transept was built with a new polygonal apse, as part of Nicholas IV's policy of improving Rome's churches. In the 15th century Cardinal guillaume d'Estouteville completed the campanile, the tallest in Rome, and Alexander VI built the gilded, coffered ceiling (completed 1498). New chapels were added to the aisles, and Gregory XIII commissioned a new portico for the façade, designed by Martino Longhi the elder. Clement X gave the apse its present arrangement, creating a monumental Baroque exterior and masking the 13th-century features. The two aisles of the present church end in cruciform spaces that open on to the Piazza dell'Esquilino. What was essentially a basilica plan was modified into a Latin cross by the chapels of Sixtus V and Paul V, which are reached through large arches, supported by coupled columns, that interrupt the rhythm of the nave colonnade. Benedict XIV transformed the building to its present appearance. Between 1743 and 1750 Fuga made every side of the complex as regular as possible and created a new solemn façade that made reference to the ancient one. It is enclosed between two symmetrical palazzi and developed on two levels: a portico with architraved openings below and a large loggia with three arched openings built against the upper part of the 13th-century facade above.
