Inaccurate tube sizing has significant clinical consequence, including failed lung isolation, poor surgical exposure, and increased risk of airway complications. Ensuring appropriate DLT fit is therefore a matter of both patient safety and clinical efficiency.\nCurrent practice relies heavily on patient height and gender to guide DLT sizing. However, evidence repeatedly demonstrates that these demographic indicators poorly predict airway anatomy, with accuracy rates often below ~50-60%. Advances imaging modalities, such as CT, have shown improved accuracy but are impractical for routine used due to cost, radiation exposure, and lack of beside availability. In contrast, ultrasound has emerged as a portable, accessible alternative for airway assessment, offering objective measurements of tracheal and cricoid diameters.\nDespite encouraging findings, the literature lacks consensus and standardization on ultrasound-guided DLT sizing. Variability in technique, training and implementations continues to limit widespread adoption of ultrasound as a non-invasive tool for airway assessment. Current studies, though promising, are restricted by small sample sizes, heterogenous methodologies, and a lack of policy-level recommendations to drive practice change.\nThe purpose of this study is to critically appraise the existing evidence on ultrasound guided DLT sizing compared to traditional methods, with the aim of determining whether ultrasound can safely provide a safer, more accurate, and standardized approach for anesthesia providers.
